We appreciate your interest! Application is made online. Below are the steps to guide you through the process.  

STEP 1. Fill out and complete the needed information in the Online Application Form.

STEP 2. The guidance office will review your submitted application and initial requirements.

STEP 3. Wait for the schedule of your exam and interview.

STEP 4.  After your interview, you will undergo a laboratory test and X-ray

STEP 5. Proceed to the Registrar's Office and submit the laboratory results, X-ray and documentary requirements

NEW STUDENT DOCUMENTARY REQUIREMENTS
A. Original and photocopy of PSA Birth Certificate
B. Original copy of Report Card with LRN
C. Certificate of Good Moral
D. Four (4) latest 2x2 ID Picture in white background

TRANSFEREE STUDENT DOCUMENTARY REQUIREMENTS
A. Original copy of Transcript of Records and Honorable Dismissal
B. Certificate of Good Moral from origin school
C. Original and photocopy of PSA Birth Certificate
D. Four (4) latest 2x2 ID Picture in white background
 

STEP 6. Proceed to the Registrar's Office for Subject Loading, Verification and Approval

STEP 7. Pay the Enrollment Fee and Wait for Enrollment Confirmation

STEP 8. ID capturing (Computer Laboratory)

STEP 9. CONGRATULATIONS! YOU ARE NOW ENROLLED.
